  • Reporting adverse events related to medical devices

  • Legal and administrative proceedings

  • Law enforcement or government requests

3. Your Rights Regarding Your PHI

You have the following rights regarding your health information:

  • Right to Access: You can request a copy of your medical records and other health information we have about you.

  • Right to Correct: You can request corrections to your health information if you believe it is inaccurate or incomplete.

  • Right to Request Confidential Communications: You can ask us to contact you in a specific way (e.g., at home instead of work).

  • Right to Limit Sharing: You may request restrictions on how we use or share your information, though we may not be able to honor all requests if required by law.

  • Right to an Accounting of Disclosures: You can request a list of disclosures of your PHI made in the past six years.

  • Right to a Paper Copy: You are entitled to a paper copy of this Privacy Policy at any time.

4. Your Choices

You may tell us how we share information in situations such as:

  • For marketing or fundraising efforts (we will always ask for written permission first)

  • For sharing your information outside of treatment, payment, or healthcare operations (requires written authorization)

You may revoke your authorization at any time in writing.

We use administrative, technical, and physical safeguards to protect your PHI from unauthorized access, use, or disclosure. While we take every precaution, no system is 100% secure.

We may update this Privacy Policy at any time. Changes will apply to all health information we maintain, past and present. The updated notice will be available in our office and on our website.
